Adult congenital and advanced heart failure training: what to look for

Two of the most specialised cardiology tracks, explained for applicants who are comparing training centers and wondering what each fellowship should offer.

Adult congenital heart disease (ACHD)

ACHD training prepares cardiologists to care for adults born with structural heart conditions, including repaired and palliated lesions, Fontan circulation, pregnancy in congenital heart disease and late complications such as arrhythmia and heart failure. A strong training centre combines a dedicated congenital clinic, imaging expertise, electrophysiology and surgical support, and regular multidisciplinary conferences.

Advanced heart failure and transplant

Advanced heart failure training covers the evaluation of patients for durable mechanical circulatory support and heart transplantation, the care of patients in cardiogenic shock, pulmonary hypertension, and long-term follow-up after transplant. Ask how many patients the centre follows, how fellows share inpatient and outpatient responsibility, and how much time is protected for research.

Why the training centre matters

Both tracks depend on case volume, multidisciplinary teams and access to surgery, intensive care and imaging. The same title can describe very different experiences, so compare what each programme actually states about its clinical scope, team and fellow responsibilities rather than relying on the name.

Certification and recognition differ by country

In the United States the American Board of Internal Medicine publishes separate certification policies for adult congenital heart disease and for advanced heart failure and transplant cardiology. Other countries recognise training through their own colleges and regulators. Confirm with the programme and the relevant body whether the training you complete will be recognised where you plan to practise.

Questions to ask a programme

Ask who supervises procedures and clinics, whether the fellowship is clinical, research or observational, how many positions are offered, whether international applicants are accepted, and what funding is provided. Compare the answers with the profile on this site and with the official page.
